Dental 32 Fresh Smiles Dental Savings Plan is a membership based in-house Dental Discount plan that lets you decide when to visit your dentist and how often. There are no restrictions on when to schedule your routine dental examinations, and no waiting periods. Your membership entitles you to save on all cleanings, fillings, crowns, and sedation services. You will receive all of this for a low annual membership fee. Our Dental Savings Plan is designed to provide greater access to quality dental care at an affordable price.

Basic Plan:
$399/year for adults, $349 for additional adult family members
$299/year for children under 12

Guidelines
- Patient’s portion of bill is due the date of service. Any services received that are not paid for at the time of service will be billed at the usual, higher fee
- Cannot be used in conjunction with another dental plan of financing program such as Care Credit (third party health care financing)
- No refunds on premiums will be issued at any time if participant decides not to utilize dental plan
- Plan fees are valid only when paid at time of enrollment.

WHAT TO EXPECT FROM YOUR TEETH CLEANING AND EXAM
Limitations
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan, dental insurance or financing program such as Care Credit (third party health care financing)
- For whitening products and dental health products purchased in the office
- For treatment which is outside of the skill set of our doctors
- For referrals to specialists
- For hospitalization or hospital charges of any kind
- For cost of dental care which are covered under medical insurance
Patients agree that Dental 32 fees stated must be paid in full at the time of service. Any service not paid for at the time of service will be billed at the office fees. Plan fees are valid only when paid at the time of enrollment. All family members must reside in the same household. This is not an insurance product.
